The Prevalence of Dizziness According to Subtypes of Migraine and Factors Related with Daily Disability

Abstract
Background: This study was aimed to assess the prevalence of vestibular symptoms and clinical factors related to dizziness according to subtypes of migraine.

Methods: Consecutive females between 18-65 years of age were diagnosed with migraine, classified as migraine without aura, with aura, chronic migraine and probable migraine from 2016 to 2018. We assessed the self-perception of handicap by Korean Dizziness handicap Inventory (KDHI) scores and self-perception of autonomic symptoms by Korean Version of orthostatic grading scale (KOGS) according to subtypes of migraine.

Results: A total of 83 female patients were enrolled; 68.7% (n=57) were had vestibular symptoms with migraine (64.2% in migraine without aura, 90.9% in migraine with aura, 77.8% in chronic migraine). Regarding the dizziness patterns, unspecific dizziness were the most common vestibular symptoms (30.1%), followed by vertigo (24.1%) and orthostatic dizziness (14.5%). Migraine with aura and chronic migraine had high mean dizziness handicap inventory scores compared with migraine without aura (37.4, 47.2 and 37.2 respectively, p<0.004). However, autonomic symptoms were more severe and frequent in migraine with aura and chronic migraine. Multiple linear regression analysis showed that intensity, frequency, and duration of migraine attack were significantly associated with self-perception of dizziness handicap.

Conclusion: The prevalence of vestibular symptoms tends to increase with duration and intensity of migraine, especially in migraine with aura and chronic migraine. Also, the autonomic symptoms occur more frequently in the ictal period of migraine. The various vestibular symptoms associated with subtypes of migraine should be considered when assessing the functional status of migraine patients.
